1. An MD order for an OT Driver Evaluation
This is required. The prescription should contain the driver’s name and reason for the evaluation, or a diagnosis. Or click here to download a copy of MD Prescription for Driver Evaluations, which can be printed and forwarded to the MD for signature.
2. Driving History
Complete the Contact Sheet and Driver Profile about driving habits and awareness. The driver being evaluated should complete the questionnaires without help from others…it’s all part of the evaluation.
3. Medical History
Provide a written list of medications taken and complete this Medical History Questionnaire.
Make a photocopy of your driver’s license.
